All referrals are reviewed within 24-48 business hours. Your office will be notified of the patient’s appointment. To avoid a delay in honoring the consult request, please carefully complete all fields.

Our online self-referral portal provides a convenient way to request non-OHIP covered services without a physician referral. Simply click the link below and complete all required fields to initiate your request.

Please note that this self-referral option is exclusively for services not covered by OHIP. For OHIP-covered services, a referral from your healthcare provider is required. We’ll review your submission within 1-2 business days and contact you promptly to schedule your appointment.
